New York State Public Health Law Article 21, Title VI, Section 2164 mandates all children must be properly immunized. The West Islip School District must exclude from attendance those students not in compliance. All entering kindergarten students are required to provide proof of required immunizations according to the New York State Requirements for School Entrance/ Attendance.
Students in the West Islip School District require vision screening and is performed in grades Pre-K, K, 1st, 3rd, 5th, 7th and 11th . Hearing screening is performed in grades Pre-K, Kindergarten, 1st, 3rd, 5th, 7th and 11th grades. Students are also screened for spinal deviation; this takes place for females in 5th and 7th grades and males in 9th grade. All new entrant students will require vision, hearing and scoliosis screening.
The New York State Education Law requires that every student have a health examination upon entrance into school at any grade level, and for each student entering Kindergarten, First, Third, Fifth, Seventh, Ninth, and Eleventh grades. Such examinations shall be acceptable if they are administered not more than twelve months prior to the commencement of the school year in which the examination is required. The school nurse is responsible for scheduling health examinations with the school physician on students who have not had their Family Physician Report completed.
Students in grades 7-12 who are interested in participating in interscholastic activities must have a sports examination completed by a school physician or by their family physician. Physical forms may be obtained in the school Health Office or downloaded here. A school nurse is assigned to each school building to implement the Health Services Program.
NO STUDENT WILL BE ADMITTED TO SCHOOL WITHOUT THE REQUIRED IMMUNIZATIONS.
Emergency Locator Cards
Each school’s Health Office maintains an Emergency Contact Card on every student to be used in the event a child becomes ill or injured during school hours. In the best interest of your child, we urge parents to complete and return these cards promptly each year. In the event any of the information changes, please notify your child’s school nurse of the current information.
